Job summary

Evolve Therapy Services in Wabasso, MN seeks a compassionate Speech Language Pathologist for our in-house Skilled Nursing Facility. This full-time, part-time, or PRN position offers a $5,000 sign-on bonus and focuses on providing quality therapy services to residents while complying with CMS standards.

The ideal candidate holds a Master's in Speech Language Pathology, CCC, and active state licensure, with ongoing CE/professional development.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active state licensure as SLP in practicing states.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals according to standards.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target list, short-term and long-term goals.
• Instruct patient, family, and staff for carryover of therapy in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in screening protocols to identify resident needs.

Education
