Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility. This position is available as Full Time, Part Time, or PRN, featuring a $5,000 sign-on bonus and tuition reimbursement.

You will assess residents, create treatment plans, and collaborate with care teams to support optimal communication and swallowing health. The ideal candidate holds a Master’s degree in Speech Language Pathology with CCC, an active state license in applicable states, and a

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant documentation in NetHealth EMR per CMS expectations.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in resident screenings to meet needs timely and with clinical excellence.

Education
