Job summary

Evolve Therapy Services in Torrington, CT is hiring a Speech Language Pathologist to serve on-site at our in-house Skilled Nursing Facility on a PRN basis. You will assess residents, plan treatment, and document care to meet CMS and NetHealth requirements.

Requirements include a Master’s in Speech Language Pathology, CCC, and an active CT license, plus ongoing continuing education to maintain licensure. Collaboration with staff ensures quality resident care.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Must have an active SLP license in the practicing state(s).
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short-, long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Provide oversight of FEES/MBS protocols and site coordination.
• Participate in resident needs screening with clinical excellence.
