Responsibilities

• Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility.
• Develops plans to address the needs of individuals and coordinates resources and services.
• Provides services for a designated caseload in community settings and facilitates transportation when needed.
• Completes and submits documentation and evaluates progress.
