Job summary

Childrens Home Society of Florida is seeking a Wellness Support Coordinator to facilitate, coordinate, and track the school health care programs referral network and processes. You will help connect CPS administration, school nurses, clinics, and health services to support students.

You will supervise medical referrals, participate in intervention teams, and help ensure timely documentation, eligibility, billing, and quality improvements within CHS’s youth-focused programs.

Qualifications

• Bachelor’s degree in Education, Social Work, Psychology, Public Health, Counseling or Nursing from an accredited university in a related field, required
• Florida Driver’s License within 30 days from hire, daily access to a reliable and insured vehicle required
• One year post-bachelor experience working with children and families, required
• Experience working in a school setting, preferred

Responsibilities

• Facilitate, coordinate and track school health care programs referral network and processes
• Serve as the point of contact between CPS administration, school nurse, medical clinic and other health-related agencies
• Supervise and coordinate all medical clinic referrals including health, dental, vision and mental health
• Record professional assessments, treatment plans and progress notes
• Participate in crisis intervention and coordinate Health & Wellness events
