Job summary

Oaklawn Psychiatric Center in South Bend is seeking a School Based Mental Health Liaison to support SBCSC students with mental, emotional and behavioral health needs. The role links families to services and coordinates with school staff and community partners.

You will work with a team, receive supervision, and participate in MTSS meetings; candidates should have a BA/BS in social services and about two years of relevant experience, plus a valid driver's license.

Qualifications

• BA/BS in social service or a human services-related field is required.
• Approximately two years' experience in a social service setting preferred.
• Valid driver's license and proof of insurance required.

Responsibilities

• Provide case management support for SBCSC students' mental, emotional, and behavioral health needs.
• Link students and families to needed services and coordinate with school staff and community partners.
• Attend MTSS meetings and support referrals to mental health or broader community supports.
• Maintain data and reports related to program participation.
