Canyon-Home-Care-and-Hospice-1 in Utah seeks a dedicated Registered Nurse to provide intermittent skilled nursing care at patients' homes. You will work under physician orders and the plan of care, supervised by the Clinical Manager, delivering high-quality, compassionate care in patients’ residence.

Responsibilities include initial evaluation, ongoing plan of care adjustments, coordination with the IDT and physicians, patient and family education, and timely documentation.

Qualifications

• Graduate of an approved school of professional nursing.
• Utah nursing license in the practicing state.

Responsabilités

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and develops the plan of care.
• Regularly reevaluates nursing needs and revises the care plan.
• Coordinates services with other IDT members and physicians.
• Completes and documents OASIS assessments and discharge summaries.
• Informs physician of changes in patient condition and needs.
• Supervises LPN/LVN and HHA/CNA as required by regulations.
• Performs total patient care and on-call duties as assigned.
