• Assist in developing, implementing, and documenting individual treatment support plans.
• Perform diverse daily tasks with persons served, including rehabilitative and behavioral interventions and scheduling.
• Provide role model support to help persons served integrate into their communities.
• Assess physical and emotional status and report changes to supervisor.
• Oversee site safety and daily living activities, including cooking, cleaning, and errands.
• Store, administer, document and dispose of medications per MAP requirements and policy.
• Complete logs and paperwork such as timesheets and intake forms.
• Respond to crises as needed.
• Maintain updated contact information for Relief Pool Coordinator or HR.
• Perform other related duties as required.
