Job summary

Qualifications

• Valid driver’s license.
• Knowledgeable of EMR systems (Cerner, OptimisPT, NetHealth and Matrix Care). If previously experience is not had, capable of learning functions within each system.

Responsibilities

• Plan, refer, screen, evaluate, carry out, and discharge patient’s treatment program in coordination with other medical professionals.
• Offer effective communication to patients regarding treatment plan and discharge goals.
• Provide quality services to all patients treated.
• Complete documentation within the EMR for the treated services.
