Responsibilities

• Provide case management and support services to youth, young adults, and families.
• Conduct outreach with schools, partners, and service providers.
• Facilitate prevention education and health promotion activities.
• Coordinate community prevention initiatives, meetings, and events.
• Maintain client and program records and assist with reporting.
• Liaise between clients, families, schools, and service providers.
