Qualifications

• Valid professional license in the state of practice.
• Graduate of CAPTE-accredited physical therapy program; foreign-trained must be deemed equivalent by a credentialing organization.

Responsibilities

• Perform evaluations, assessments, and treatments per the care plan.
• Complete documentation promptly and accurately.
• Follow established policies and scope of practice.
• Identify priorities in patient care and exercise sound judgment.
• Complete orientations, trainings, and in-services on time.
• Perform other related duties as directed by supervisor.
