Job summary
Covenant Living and CovenantCare at Home seeks a licensed Physical Therapist to provide comprehensive therapy services in home settings and contribute to care planning. The role emphasizes mobility assessment, program initiation, and caregiver instruction within a senior-focused care model.
The candidate will evaluate environments, coordinate discharge planning, and participate in peer consultations, with at least 2 years of experience and state-appropriate licensure.
Qualifications
- Bachelor’s Degree in Physical Therapy from an accredited program.
- Master’s Degree in Physical Therapy preferred.
- Certification: PT License and CPR, as required by state.
- Experience: 2+ years in therapy.
Responsibilities
- Assess therapeutic, rehabilitative, functional status and develop total care plan.
- Evaluate home environment and perform mobility/ROM assessments.
- Direct physical therapy treatment and initiate programs.
- Instruct patients and caregivers; participate in case conferences.
- Assist in discharge planning and peer consultation; provide home-therapy guidance.
