Job summary

Gurwin Jewish Nursing & Rehabilitation Center in Commack, NY is seeking a full-time Physical Therapist to join our compassionate care team. The ideal candidate holds New York State PT licensure, has completed an APTA/AMA approved program, and brings at least one year of clinical experience to develop patient-centered therapy plans and discharge goals.

You will collaborate with a multidisciplinary team on a 36-acre campus delivering short and long-term rehab, memory care and palliative services

Qualifications

• New York State PT license required.
• Graduate from a PT program approved by the American Physical Therapy Association or the American Medical Association.
• Minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform PT procedures competently and independently.

Responsibilities

• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Complies with all regulatory criteria.
• Provides professional leadership and clinical guidance and expertise to team members.
• Attends regular continuing education programs.
• Adheres to all aspects of Customer Service, including empathy, courteous communication, and respect for others.
