Job summary

Qualifications

• Graduate of an accredited physical therapy curriculum with a Bachelor, Master or Doctoral degree and PT certification or foreign trained equivalent.
• California Physical Therapist License required at hire.
• National Provider Identifier required at hire.
• Basic Life Support (BLS) certification required.
• Knowledge of federal, state, and local requirements.
• Ability to work in a Labor/Management Partnership environment.

Responsibilities

• Deliver direct patient care in accordance with the Physical Therapy Practice Act and Kaiser Permanente HealthPlan.
• Provide accurate and timely documentation of services per state and federal requirements and relevant policies.
• Participate in multidisciplinary clinics, patient care rounds, and case management conferences.
• Refer patients to appropriate medical providers when care is outside of scope of practice.
• Instruct patient education classes or groups as requested by management.
• Provide department in-service as requested by management.
• Assist in orientation and training of employees as needed; may serve as clinical instructor.
• Provide consultation to staff and mentor for staff.
• Participate in performance improvement projects or other committees as requested.
