Responsibilities

• Conduct client assessments to identify history, presentation, problems and treatment needs.
• Perform ongoing assessments of chemical dependence services within the program.
• Develop and review initial treatment plans and progress notes.
• Lead group, individual or family sessions using evidence-based modalities.
• Educate clients and families on substance abuse and psychiatric issues.
• Ensure timely, clear documentation of client care and incidents.
• Provide crisis intervention and aftercare planning.
• Coordinate with insurance carriers and referral sources as needed.
• Provide case management and liaison duties for clients.
