Job summary

Manchester Center for Rehabilitation and Healing is seeking an Occupational Therapist to join our team in Morrison, Tennessee. The OT will assess referred patients, provide direct patient care, and develop individualized treatment plans under physician direction.

The role requires a valid OT license in the state, experience with documentation and discharge planning, and collaboration with interdisciplinary teams to deliver patient-centered care.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid license and registration to practice in the state.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Punctuality and professionalism in scheduling.
• Compliance with confidentiality, HIPAA and Residents' Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Set realistic goals, document patient condition, and educate patients and families in appropriate therapy methods.
