Qualifications

• Minimum Years of Experience (to be filled in by hiring manager) of experience as an Occupational Therapist
• Level of Education (to be filled in by hiring manager) in Occupational Therapy from an accredited program
• State Licensing (to be filled in by hiring manager) and certifications in Occupational Therapy

Responsibilities

• Provide occupational therapy services to residents in a skilled nursing and rehabilitation setting, including assessing, planning, implementing, and evaluating treatment plans
• Develop and implement individualized treatment plans to address residents' needs and goals
• Collaborate with other healthcare professionals for comprehensive care
• Conduct thorough assessments to identify residents' strengths, weaknesses, and limitations
• Utilize evidence-based practices to promote functional independence and quality of life
