Job summary

Confluence Psychological Services, LLC is seeking an Occupational Therapist to manage a caseload of young adults within the Confluence Transitions program. The role focuses on activities of daily living, executive functioning, and independence, with weekly sessions and ongoing progress tracking.

The OT will evaluate needs, develop treatment plans, and educate families, while collaborating with mentors and direct care staff to support the therapeutic model.

Qualifications

• State of Vermont OT licensure required or clearly eligible for licensure.
• Experience or interest in working with emerging adults (late teens to mid-20s).

Responsibilities

• Weekly individual OT sessions and group sessions with participants.
• Assessment, treatment and discharge planning.
• Ongoing assessments and check-in sessions for participants.
• Collaborate with direct care staff to support therapeutic process.
• Review medical history and assess physical, mental, and cognitive capabilities.
• Create personalized, goal-oriented treatment plans.
• Teach daily living skills (e.g., dressing, eating, bathing, cooking).
• Educate families and caregivers on support strategies.
• Monitor progress and adjust care plans as needed.
• Participate in weekly clinical team meetings.
• Engage in marketing events, presentations, and publication writing.
