Job summary

The Christ Hospital Home Health Care is seeking a licensed Occupational Therapy Assistant to provide skilled OT services in patients' homes under the supervision of an OT. The role includes educating patients and families, documenting care, and coordinating with the care team.

Responsibilities include driving to patient locations, adhering to care plans, and maintaining licensure and professional standards. Monday–Friday, 8:00 AM–5:00 PM with a defined territory across multiple communities.

Qualifications

• OTA license in the state of employment.
• At least 1 year of OTA experience; home health care experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Provide skilled occupational therapy services per the plan of care.
• Educate patient and family on treatment and safety.
• Complete and document all assessments, treatment notes, and education per Medicare/Federal guidelines.
• Communicate with physician, therapist, and care team; participate in case conferences.
• Travel to patients' homes as scheduled and document visits within required timelines.
