Responsibilities

• Evaluates patients using appropriate tools per history and diagnosis.
• Collaborates with care team to ensure safe, accurate evaluations.
• Sets realistic, measurable long- and short-term goals with patient and family.
• Plans and delivers treatment per standards of care.
• Acts as clinical resource to staff and committees.
• Maintains HIPAA confidentiality and follows safety protocols.
• Maintains regular and predictable attendance.
• Communicates with patients, families and co-workers.
