Job summary

Baywood Post-Acute in Campbell, CA, a 45-bed, 5-star skilled nursing facility, is seeking an Occupational Therapist to help patients regain independence through personalized rehabilitation.

You will assess patients with neurological and orthopedic conditions, design individualized treatment plans, and collaborate with physicians, physical therapists, and speech therapists to enhance daily living and community participation.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy from an accredited program.
• Current and valid OT license to practice in the United States.
• Experience in rehabilitation settings with neurological and orthopedic patients.
• Knowledge of sensory integration, physical therapy, and functional rehab.
• Excellent communication and interpersonal skills with patients, families, and teams.

Responsibilities

• Conduct thorough patient evaluations of physical, cognitive, and sensory abilities.
• Develop individualized treatment plans with adaptive techniques.
• Deliver rehab interventions for geriatric impairments, stroke, TBI, CP, and ortho injuries.
• Monitor progress and adjust plans to optimize outcomes.
• Collaborate with physicians, PTs, SLPs, and other professionals.
• Educate patients and families on exercises and adaptive equipment.
• Maintain accurate documentation per regulatory standards.
