Elara Caring in Shawnee, OK is seeking an Occupational Therapist to join our home health team, providing high-quality care to patients in their homes. A $2,500 sign-on bonus is offered to eligible candidates.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.

Responsabilités

• Evaluates the occupational therapy patient to determine rehabilitation needs and develops a plan of care.
• Implements and documents Plan of Care to ensure quality and continuity of care.
• Performs initial evaluations and subsequent re-evaluations; follows OASIS/PPS guidelines.
• Communicates findings to supervisor and care team; coordinates with patient, family, and resources.
