Elara Caring is seeking a proactive Occupational Therapist to join our home health team. The role involves evaluating patients, developing and implementing plans of care, and coordinating with physicians to ensure quality, safe discharge planning.

You will educate patients and families on daily living activities and adaptive equipment while traveling to patient homes. We offer a $5,000 sign-on bonus, competitive compensation, benefits, and opportunities for professional growth.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.

Responsabilités

• Evaluates the occupational therapy patient to determine rehabilitation needs and develops a plan of care.
• Implements and develops the Plan of Care to ensure quality and proper discharge planning.
• Writes physician orders and provides care according to physician’s orders and established guidelines.
• Instructs patients and families in activities of daily living and adaptive equipment usage.
• Counsels and instructs patient and family in meeting therapy needs and safety.
