Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist to deliver home-based OT services, enhancing daily living skills and functional mobility for patients in their homes.

The role requires state licensure, a proven ability to work with families, and coordination with physicians under an established plan of care. A sign-on bonus is offered and benefits support ongoing professional development.

Qualifications

• Graduate of an approved school of Occupational Therapy and licensed as an OT in the practicing state.
• Minimum of one-year experience preferred; can be waived based on prior leadership/clinical experience with VP/Regional approval.
• Medicare home health experience preferred.

Responsibilities

• Assist patients in improving daily living skills and basic motor functions for home-based care.
• Provide skilled OT services under physician orders and established plan of care.
• Educate and collaborate with families and health care team to support patient progress.
