Job summary

ACMC Family Home Health, part of LHC Group and UnitedHealth Group family, seeks an Occupational Therapist to join our home health team. You will assess and evaluate patient care needs related to functional status, ADLs, and home safety, and collaborate with the care team to develop therapy plans.

Responsibilities include performing patient assessments, documenting CMS-relevant status, and adapting care for diverse patients.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle

Responsibilities

• Provides service within the scope of practice as defined by state laws and plan of care.
• Evaluates patient's functional status and CMS-relevant needs; consults with physician.
• Observes, records and reports patient response to treatment and condition changes.
• Conducts patient assessments evaluating function with diagnostic tests.
• Ability to work in any home situation regardless of client background.
