Job summary

Blue Stone Therapy is seeking an Occupational Therapist (OTR) to provide rehabilitation services to adult and geriatric patients at Mapleton Community Home and Whispering Creek skilled nursing facilities in MN. You will assess, plan, and implement treatment programs to improve ADLs, cognition, and functional independence.

The role requires licensure (or eligibility) in Minnesota), strong clinical reasoning, and the ability to collaborate with an interdisciplinary team in a post-acute setting.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide skilled occupational therapy treatment focused on functional and measurable outcomes.
• Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning.
• Supervise Occupational Therapy Assistants in accordance with regulatory and company standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
