Qualifications
Graduate of an accredited Occupational Therapy program.
Current Occupational Therapist license (or eligibility for licensure) in applicable state.
Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
Strong clinical reasoning, communication, and organizational skills.
Commitment to patient-centered care and professional standards.
Responsibilities
Perform occupational therapy evaluations and develop individualized plans of care.
Provide skilled occupational therapy treatment focused on functional and measurable outcomes.
Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning.
Supervise Occupational Therapy Assistants in accordance with regulatory and company standards.
Maintain appropriate patient schedules per established guidelines.
Complete timely, accurate documentation to support quality care and reimbursement requirements.
Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
