Continuum Therapy Partners seeks an Occupational Therapist for Meadow View Nursing Center in Berlin, PA. The position is full-time with daytime hours, offering a full benefits package and licensure support.

A Master’s or Doctorate in OT is required, with an active state license and NBCOT certification. The ideal candidate has 1 year of experience preferred and will evaluate and develop individualized treatment plans, document progress, and educate patients and families on adaptive equipment and

• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
