Job summary

Elwyn is hiring a Mental Health Case Manager in Bangor, ME. The role focuses on coordinating health care, behavioral health treatment, and social services for adults and youth in community settings. You will work with multidisciplinary teams, families, and community partners to ensure timely access to services.

The position requires a degree in a human services field (preferred) and MHRTC certification. A valid driver's license and 3 years of relevant experience are expected; CPR/first aid and

Qualifications

• Bachelor's in Human Service field preferred but not required.
• MHRTC Required.
• Three years' experience with adults in behavioral health.
• Nonprofit human service experience preferred.
• Teamwork and strong attention to detail required.
• Proficiency with Word, Excel, Outlook; PowerPoint/Access a plus.
• Driving with valid license and clean record required.

Responsibilities

• Identify service needs during intake and evaluation.
• Coordinate treatment plan with team, family, and care partners.
• Assist residents and families in obtaining services (housing, recreation, education).
• Enter incidents into reporting system to maintain compliance.
• Maintain resident financial log and manage funds.
• Monitor caseload, intake, and evaluation schedule.
• Coordinate SSI/DHS authorizations and benefits; liaise with court reps.
• Perform CPR, first aid, and crisis intervention per protocol.
• Perform other duties as assigned.
