Job summary

Aroostook Mental Health Center (AMHC) in Maine is seeking a Full-Time Adult Community Case Manager to empower adults living with long-term mental health challenges to achieve greater independence and wellness.

The role involves building relationships, coordinating services across medical, behavioral health, housing, employment, and education, and providing person-centered support. Travel within the service area is required, with occasional evening hours as needed.

Qualifications

• Associate's Degree or equivalent in Human Services is required.
• Bachelor's degree preferred.
• MHRT-C or provisional certification required.
• Personal vehicle and valid driver's license required.

Responsibilities

• Build therapeutic relationships with clients using a recovery-oriented, person-centered approach.
• Coordinate services based on each client's plan and connect to medical, behavioral health, housing, employment, financial, educational and community resources.
• Facilitate and support psychoeducational and wellness activities.
• Provide crisis intervention and ongoing support as needed.
• Maintain accurate clinical documentation and participate in team meetings and quality initiatives.
