Job summary

The H.U.F. Center in Virginia seeks a compassionate Case Manager to support adults with mental health and co-occurring disorders in a psychosocial day support setting. This hands-on role emphasizes direct member support, timely documentation, and collaboration with a multidisciplinary team.

QMHP certification and a related bachelor’s degree are required; occasional Saturday shifts may be needed. Benefits include health, life, PTO, and a 401(k).

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.
• QMHP-A/C certification required (Virginia)

Responsibilities

• Provide direct support and case management services to assigned members
• Observe, engage, and interact with members daily
• Complete timely documentation within 24 hours (daily notes, ISP, reports)
• Facilitate group and individual psychosocial interventions
• Assist with daily program operations and safety
• Collaborate with team members, caregivers, and external providers
• Support members with ADLs/IADLs as appropriate
