Hampton-Newport News Community Services Board is hiring a full-time Adult Mental Health Case Manager to provide behavioral health case management, care coordination, advocacy, and supportive services to adults in the Hampton and Newport News communities.

The position requires a Bachelor's-level human services professional with case management experience who can assess client needs, develop and monitor treatment plans, and connect individuals with community resources.

Qualifications

• Bachelor's degree in human services or related field.
• Experience in case management.
• Ability to assess client needs and develop treatment plans.
• Ability to respond to crisis situations.

Responsibilities

• Provide behavioral health case management, care coordination, advocacy, and supportive services to adults.
• Coordinate services, develop and monitor treatment plans, and connect individuals with community resources.
• Provide supportive counseling and advocacy; respond appropriately to urgent or crisis situations.
• Work as part of a collaborative behavioral health team to support adults in the Hampton and Newport News communities.
