Job summary

NaphCare, partnering with Everhealth, joins the Arizona Department of Corrections - Perryville in Goodyear, AZ as a Release/Medical Discharge Planner. This role focuses on discharge planning and coordinating post-release care for inmates to ensure seamless transitions back into the community.

You will collaborate with correctional officers, medical staff, and community resources to address medical, mental health, and social needs while maintaining thorough records and ensuring compliance with

Qualifications

• Bachelor's degree in human services, nursing, social work, or healthcare.
• Active BLS CPR (AHA) certification.
• Experience with discharge planning and coordination with community services.
• Excellent communication with patients, staff and physicians.

Responsibilities

• Discharge planning for inmates released from the facility.
• Identify post-release needs and create individualized plans.
• Coordinate with healthcare professionals, social workers, and community resources.
• Maintain accurate records of discharge plans and follow-up activities.
• Provide information and support to inmates and families on discharge options.
• Liaise between the facility and community agencies to facilitate referrals and services.
• Assist patients with medication management plans and follow-up arrangements.
• Coordinate care with shelters, employment and counseling services.
