Job summary

Excelsior Care Group is seeking a Licensed Practical Nurse (LPN) for our West Volusia Healthcare & Rehabilitation facility in Deltona, FL. This full-time role involves providing comprehensive nursing care to residents, including medication administration, wound care, IV therapy, and coordinating with the care team to support optimal resident outcomes.

Qualified candidates hold a Florida LPN license, CPR certification, and have at least 2 years in long-term care.

Qualifications

• Graduate of an accredited school of nursing.
• Valid LPN license in Florida.
• CPR certification required.
• Minimum 2 years of experience in long-term care or related settings.
• Strong clinical assessment, critical thinking, and teamwork skills.

Responsibilities

• Administer and record medications and treatments.
• Assist physicians with diagnostic procedures and treatments.
• Observe vital signs and report residents' condition.
• Provide direct nursing care including wound care and IV therapy.
• Assist with admissions, discharges, and transfers of residents.
• Document care in compliance with regulations.
• Communicate with residents, families, and the care team.
• Ensure infection control and facility policy compliance.
• Respond to emergencies and implement interventions.
• Support CNAs and nursing staff in care delivery.
• Participate in care plan meetings and resident assessments.
• Educate residents and families on health conditions and self-care.
