West Delray Nursing and Rehabilitation Center in Delray Beach, Florida, is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a full-time basis. This role offers a competitive hourly wage up to $32 plus shift differentials and a supportive, team-oriented environment.

You will administer medications, follow care plans, and coordinate with the nursing leadership to ensure high-quality resident care, while maintaining HIPAA compliance and documenting interventions

Qualifications

• Active Florida LPN license is required.
• Prior Skilled Nursing Facility experience preferred.
• Commitment to teamwork and high-quality resident care.
• Maintain HIPAA-compliant documentation and confidentiality.

Responsibilities

• Administer medications and treatments per regulations.

Salary: Up to $32 per hour + shift differentials

Key Responsibilities:

• Administer medications and treatments in accordance with federal, state, local, and facility regulations

• Work closely with the Director of Nursing and Assistant Director of Nursing to ensure excellent resident care

• Order medications, supplies, and equipment as needed

• Follow individualized care plans for each resident

• Perform wound care and related medical treatments

• Document observations, interventions, and evaluations accurately and promptly

• Maintain resident and staff confidentiality in compliance with HIPAA

Requirements:

• Active LPN license in the State of Florida

• Prior experience in a Skilled Nursing Facility preferred
