• Master's Degree in Mental Health Counselling or Marriage and Family Therapy.
• Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW).
• 2–5 years’ experience, preferably with children/adolescents.
• Experience with electronic health records preferred (e.g., NextGen).

Responsibilities

• Coordinate social service planning and related services to support patients and families.
• Provide intake and patient advocacy services.
• Collaborate with physicians, staff, patients, families, and community organizations.
• Maintain required documentation and meet regulatory standards.
