Job summary

Community Health Systems is seeking a Licensed Master Social Worker (LMSW) at DeTar Hospital in Victoria, TX. Qualifications

• Master's Degree in Social Work (MSW) or Psychology required.
• 2-4 years of experience in a healthcare or clinical social work setting preferred.

Responsibilities

• Conducts age-appropriate psychosocial assessments for patients across the lifespan.
• Assesses psychosocial factors and barriers to recovery and discharge.
• Provides emotional support, counseling, and crisis intervention to patients and families.
• Identifies social, emotional, and financial needs and facilitates referrals.
• Develops individualized treatment and discharge plans with the care team.
• Coordinates transition of care with home health, hospice, and long-term care.
