Job summary

Garfield Counseling Center, Inc. in Chicago is seeking a Full-Time LCSW to deliver comprehensive behavioral health services to adults with substance use disorders and co-occurring disorders.

Reporting to the Project Director, you will perform assessments, treatment planning, counseling, crisis intervention, care coordination, and recovery support as part of a 1.0 FTE Year 1 position. This role collaborates with a multidisciplinary team to integrate behavioral health with MAT, peer recovery

Qualifications

• Licensed LCSW in Illinois in good standing.
• Minimum two years of behavioral health experience with SUD/COD.
• Experience with evidence-based practices and multidisciplinary care coordination preferred.

Responsibilities

• Conduct screenings, assessments, and develop individualized treatment plans.
• Provide individual and group counseling using CBT, MI, and trauma-informed approaches.
• Coordinate care with medical and recovery teams to ensure integrated services.
• Monitor progress, update treatment plans, and provide crisis intervention and referrals.
• Support workforce readiness and discharge planning.
