Job summary

TURN is seeking an Intake & Eligibility Coordinator to support access to medical, educational, social, vocational, housing, and more for program goals in San Diego. The role involves referrals to the next level of care, coordinating with health providers, and advocating for clients’ best interests.

The position requires a Master’s degree with CA BBS licensure, a valid CA driver’s license, and experience in mental health services.

Qualifications

• Master’s degree and CA BBS registration or license required.
• Valid CA driver’s license and current auto insurance per contract requirements.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide assessments, mental health evaluations, family-individual-group therapy, plan development, case consultation, crisis intervention, discharge placement, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Conduct initial clinical assessments and eligibility screenings for prospective members.
• Complete annual assessment updates and maintain accurate, timely clinical records.
• Collaborate with members to identify strengths, goals, and support needs using a recovery-oriented approach.
• Coordinate referrals and communicate with internal and external partners as appropriate.
• Support a welcoming, strengths-based environment that promotes hope, dignity, member choice, and community inclusion.
• Ensure compliance with program, funding, and documentation requirements.
• Participate in regular clinical supervision and consultation to support quality care and professional development.
