CarePlus NJ, Inc. in Rochelle Park, NJ is seeking a Case Manager for its Housing Supportive Services program. You will provide case management to NJ Family Care members at risk of homelessness, focusing on housing, tenancy maintenance, and sustainability.

You will assist with screenings, intake, eligibility, and community outreach to maintain housing stability. The role involves crisis intervention, documentation, and coordinating resources as needed.

Qualifications

• Bachelor's degree in health, psychology, counseling, social work, education or related behavioral health field.
• Experience in case management activities.
• Bilingual Spanish strongly preferred.
• 1–2 years' experience with homeless or high-need populations and/or SMI/COD/SUD.
• Valid driver's license and insurability by the agency.

Responsabilités

• Act as a first responder addressing crises with psychological and environmental factors.
• Work holistically to meet physical, social and emotional needs and ensure discharge plans.
• Create care plans with clients and coordinate resources as needed.
• Assist individuals with housing assistance.
• Complete comprehensive assessments and identify resources.
• Document all services in the clinical record with time and date.
• Link clients to community resources.
• Maintain client confidentiality and ethical boundaries.
• Provide team support and participate in performance improvement processes.
• Engage in care coordination and follow up on referrals.
• Attend MDT meetings and prepare billing documentation as needed.
