Job summary

Bloom Healthcare, a Colorado leader in population health management, home-based primary care, and hospice, is seeking a Social Worker to join our growing Hospice Team and work 7-10 visits per month. Bloom Social Workers are cross trained to provide care in several different areas including home visits, imminent and admission visits, as well as palliative care.

Qualifications

• Master's degree in social work from an accredited school
• 1–3 years of palliative care or hospice experience
• Ability to assess emotional and social needs of patients and families
• Excellent communication and interpersonal skills
• Ability to collaborate with an interdisciplinary team
• Organizational skills and time management
• Professional image, positive attitude and self-motivation
• Comfortable maintaining professional boundaries
• Proficient computer skills
• Knowledge of community resources including financial benefits

Responsibilities

• Develops and maintains therapeutic relationships
• Provides comprehensive psychosocial assessments
• Develops and maintains care plans
• Provides supportive counseling
• Facilitates transitions between care environments
• Mobilizes community resources
• Delivers patient/family education
• Documents promptly and accurately
• Ensures patient safety
• Works 50% of caseload in home-based settings
