Job summary

Compass Healthcare Consulting & Placement is conducting a search for a Hospice Medical Social Worker position making visits to patients at home in Brooklyn, NY. Qualified candidates will have prior Hospice or Home Care Social Worker or bereavement counseling experience as well as the ability to travel to patient locations.

Driver's license and access to vehicle, required. The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients

Qualifications

• MSW from an accredited school of social work and one year related experience.
• LMSW or LCSW licensure required.
• New York State driver's license required.
• Minimum one year bereavement counseling experience preferred.
• At least one year Hospice or Home Care experience preferred.

Responsibilities

• Provide medical social services, psychosocial and bereavement counseling to hospice patients and families at home.
• Travel to patient locations is required.
• Visit areas available in Brooklyn, NY.
