Job summary

Gentiva is seeking a Hospice Social Worker (BSW) to provide compassionate psychosocial support to patients and their families as they navigate the hospice journey. You will assess needs, guide families through illness and loss, and connect them with education and community resources.

You will collaborate with an interdisciplinary hospice team to address holistic care, monitor changes, and advocate for patient dignity and comfort in challenging moments.

Qualifications

• Bachelor’s Degree in Social Work (BSW) from a CSWE-accredited school.
• Licensure as required by the state of employment.
• At least 1 year of experience in healthcare or hospice settings.
• Knowledge of psychosocial dynamics of illness, grief, and end-of-life care.

Responsibilities

• Provide psychosocial support to patients and families per a plan of care.
• Assess social, emotional, financial and environmental needs.
• Guide patients and families through illness and loss with professional judgment.
• Offer counseling, education, and referrals to resources.
• Collaborate with the hospice interdisciplinary team to address whole-person needs.
• Monitor changes and adjust services accordingly.
• Promote dignity, respect, and advocacy for every patient and family.
