Job summary

Beaumont Home Health in Michigan is seeking a Registered Nurse Case Manager to lead the assessment and plan of care for patients in a home health setting. You will coordinate multi-disciplinary teams, educate patients and families, ensure safety, and document thoroughly to meet state and Medicare guidelines.

The RNCM will participate in on-call rotations, drive outcomes, and manage physician orders while maintaining compliance.

Qualifications

• Registered Nurse with current license in the state of employment.
• One year of RN experience in acute care; home health preferred.
• Strong communication and teamwork abilities.

Responsibilities

• Oversees overall care from start to end of episode.
• Updates doctor orders in patient chart.
• Notifies physician of significant changes in patient condition.
• Coordinates multi-disciplinary care as needed.
• Provides skilled nursing care as ordered by the physician.
• Documents treatment and plan of care per Medicare guidelines.
• Participates in on-call requirements and team meetings.
