Team Select Home Care in Denver seeks a Registered Nurse (RN) / Case Manager Support to oversee skilled nursing care in patients' homes per physician orders and the plan of care. The role blends hands-on nursing with case management responsibilities on an intermittent basis.

Requires current RN license, CPR certification, and driving to patient homes in the Denver area. Competitive pay and benefits offered.

Qualifications

• Must hold an RN license in the applicable state.
• One year nursing experience; home health preferred.
• CPR certification required.

Responsibilities

• Develops and implements patient-centered home care plans under physician supervision.
• Provides case management for assigned patients and coordinates with families.
• Submits required data sets (OASIS, PAR, etc.) on time to state/federal payers.
• Performs initial and ongoing nursing assessments and reevaluations.
• Administers medications per orders and maintains documentation.
• Educates patients and families on home health strategies and resources.
• Participates in case conferences and communicates changes to physicians and team.
• Maintains confidentiality and adheres to agency policies.
