Job summary

Adoration Health in Lancaster, SC seeks a dedicated Home Health Physical Therapist to deliver 1:1 patient care in the comfort of patients' homes. You will evaluate, treat, and develop plans to maximize independence, coordinating with the care team and families.

The role is full-time with a sign-on bonus of $10K for external candidates, offering flexible scheduling, strong support from a multidisciplinary team, and opportunities for professional growth within a growing home health organization.

Qualifications

• Graduate of a CAPTE-approved physical therapist education program.
• Current Physical Therapist licensure in state and CPR certification.
• Two years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.

Responsibilities

• Perform PT evaluation and treatment of patients based on physician orders, including adaptive equipment.
• Establish measurable goals and treatment plans to maximize independence and function.
• Maintain communication among treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services for accurate billing.
• Supervise PTAs and PT aides in direct patient care as needed.
