Job summary

Adoration Health in Lancaster, SC is seeking a Home Health Physical Therapist to provide 1:1 patient care in home settings. You will evaluate and treat patients per physician orders and set measurable goals for independence.

Qualifications

• CAPTE-approved Physical Therapist Education program completion.
• Current state licensure as Physical Therapist and CPR certification.
• Two years of adult physical therapy; home health experience preferred.
• Excellent observation and communication skills; basic math skills.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to improve independence.
• Maintain communication among treatment team, physician, patient and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT aides per state practice act.
