Job summary

Penn Medicine At Home is seeking an experienced Physical Therapist to deliver in-home care across Lancaster County. You’ll assess patients in their homes, design and implement individualized treatment plans, and work with a diverse clinical team to optimize outcomes.

This role emphasizes clinical autonomy, one-on-one patient interaction, and the opportunity to grow through our Clinical Ladder program while maintaining the highest standards of care in a home-based setting.

Qualifications

• Bachelor's degree CAPTE-accredited (MS or DPT preferred)
• Licensed to practice physical therapy in Pennsylvania
• BLS/CPR as a healthcare provider per the American Heart Association
• Valid Driver's License
• Car Insurance

Responsibilities

• Provide skilled therapy to patients in their homes and assess physical abilities, restore maximum function and alleviate pain
• Collaborate with interdisciplinary team to plan and carry out individualized PT treatment programs
