Job summary

Alpine Care Home Health in Columbus, OH is seeking a Physical Therapist Per Visit to deliver skilled PT services in patients' homes. You will assess mobility, design care plans, and work with a multidisciplinary team to promote independence and safe aging in place.

This role requires an active state license, CPR certification, and a valid driver's license. You’ll supervise home health aides when needed and participate in ongoing quality improvement and patient education.

Qualifications

• Current, unrestricted state physical therapy license.
• Active CPR certification.
• Valid driver's license and reliable vehicle.
• Minimum one year home health PT experience.
• Strong clinical judgment and ability to assess patient needs.
• Proficiency with standard software (email, word processing, spreadsheets).

Responsibilities

• Admit patients eligible for home care within 24–48 hours.
• Develop and update patient care plans addressing identified problems and goals.
• Provide physical therapy services per plan of care.
• Supervise home health aides when applicable.
• Communicate patient progress to physician and supervisor; coordinate with care team.
• Perform Medicare-requisite OASIS assessments as required.
