Job summary

Kaizen Lab Inc. is seeking a licensed Physical Therapist to provide skilled home health services in Massachusetts. You will assess, treat, educate patients and caregivers, and collaborate with physicians to implement treatment plans in a home setting.

The role requires MA licensure and CAPTE-accredited PT education, with responsibility for evaluating progress, updating care plans, and ensuring adherence to physician orders during discharge planning and coordination with the care team.

Qualifications

• Licensed Physical Therapist in MA or eligibility.
• Graduated from CAPTE-accredited physical therapy program.
• Passed state licensure examination as required by the state.

Responsibilities

• Provide skilled physical therapy in accordance with the physician’s plan of treatment.
• Assist the physician in evaluation and assessment of the client’s level of functioning.
• Treat clients to relieve pain, develop and restore function and maintain maximum performance.
• Observe, record, and report client’s reaction to treatment and any changes to the physician and managers.
• Educate client/caregiver and health care team in PT care and use of devices and equipment.
• Provide staff in-service education as directed by Clinical Manager.
• Assess and reevaluate client needs; develop and revise the plan of care.
• Initiate diagnostic, preventive and rehabilitative procedures as appropriate.
• Assist with plan of care continuity across multidisciplinary teams.
• Promote coordination of care through interdisciplinary case conferences and communication.
• Maintain individualized client care progress records per policy.
• Participate in the agency’s quality improvement program.
• Stay updated with physical therapy trends for service provision.
• Participate in staff meetings and utilization reviews.
• Supervise the Physical Therapy Aide at least every 30 days.
